Oral surgery :: Dental Remplantation

The term "replantation" is derives from Latin and in translation means "replanting", ie. the tooth, which for some reason is extractable, is first extracted in the most atraumatic way, the root canals are treatment and filled, it is sealed and returned to its alveolus in the jaw bone from which it is extracted.
There are three ways to fix a replanted tooth:
- By a partial tooth splint, spanning two adjacent teeth next to the tooth to be implant, with each tooth individually attached to the splint by a wire ligature. Nowadays, the toot splint can be made with orthodontic wire 0.9 mm, which is attached to the surfac-
-Through a wire splint inside the adjacent tooth and in the tooth to be implant. Fix with a photo polymer. This is done when on the next tooth or two adjacent teeth of the replanted tooth have fillings or caries on them. Otherwise, it is not justified to break their integrity in order to fix the tooth that will be replanted.es of the teeth with liquid photo polymer.
- Through circumferential ligature, which is only possible with the teeth of the mandible.
The method of replantation is a method practiced in ancient times, which referred to by archaeological finds in Egypt, Mexico and others. Abul Qiyazim, a well-known Arab scholar, claims "... it is too easy for the skilled doctor." How did this actually happen? I will set an example with ancient Egypt. The pliers with which the Egyptians extracted teeth in shape and type resemble the beak of a pelican, which is why they called "pelican type pliers". They oxidized, not nickel-plated.

Another version of pliers in ancient Egypt. Surprisingly lots of resemble today's pliers called "bayonet".
After extracting the tooth, the ancients with hot platinum wire treated the root canals, which after that filled with natural epoxy resins. The cavity of the teeth sealed with gold dust. Then they were replanted the tooth into its alveolus and attached it with gold wire or a splint to the adjacent teeth.

In addition, here is how they made holes in the teeth - a flint fixed on of a stick, around which the bowstring were wrapped, and through the bow's movements back and forth, a rotary motion obtained. This method is identical to the one used to produce the fire.
The archeological findings date back to 4000 years. The replantated teeth were fixed the same way.
Nowadays, the method of treating teeth through implantation is a well-forgotten old method and is practically nowhere practiced. It should be noted, however, that this is the last chance a tooth can be saved. According to Prof. Kadankov, whose dissertation is for the replantation, a single implanted tooth has a functional capacity of 8 to 15 years. During this time, the organism slowly absorbs its root as the roots of the milk teeth are absorbed, and in fact at one point the tooth remains rootless. This is why we lose it 8 - 15 years after the replantation. We cannot disagree that this is a serious success in this kind of treatment.
My practice shows identical results.
How we do it in my office:
We extract the tooth very carefuly, clean the carious mass in the cavity, scrape the root surface to smooth, freeing it from the collagen filaments with which the tooth was attached to the bone, washing with oxygen water, and grasping the tooth with a gauze soaked in desinfection solution. We hold the tooth like this while treating the root canals in the usual endodontic manner using machine rotary tools. Filling them with cement is carbofine with timol. We put gutta-percha pins. The apex is cut, and then we seal the cavity. Finally, the tooth is sealed without a pin or pin according to the extent of its destruction. We then treatment the alveola until it is filled with fresh blood. This is followed by implantation and fixation in one of the above ways.

A classic example of a replanted tooth. The tooth had a huge granuloma. The splint has been removed and the tooth has integrated  well into the bone.

Replanted molar, which sealed with a pin and fixed with a circumferential ligature.

Replanted premolar fixed with circumferential ligature. The adjacent tooth is intact in the front and the posterior tooth - with a crown. Circumferential ligature is the preferred method for fixing the implanted tooth.

Replanted molar fixed with circumferential ligature. The adjacent tooth at the back is missing and the anterior tooth is with obturation. Again, a circular ligature is the better choice for fixing. The fillings in the apex are clearly visible.

Due to numerous hacking attacks on previous versions of my site, many of my clinical case images have been lost. In the near future, I will show replanted teeth of the upper jaw and other fixation methods.
